The New York City Police Department is seeking the public’s assistance in locating the following missing male who resides within the confines of the 83rd Precinct in Queens.

Details are as follows.

Juan Velazquez
392 Cornelia Street
Brooklyn, NY 11237

It was reported to the police that Juan Velazquez was last seen Monday, March 30, 2020, at approximately 1621 hours, at Long Island Jewish Hospital, located at 270-05 76th Avenue in Queens.

The missing is described as an 82-year-old me, Hispanic, being approximately 5’4″ tall, weighing 170 lbs., with a thin build, gray hair, and brown eyes.

He was last seen wearing a blue hat, gray jacket, beige cargo pants, and brown shoes. He is wearing a medical alert bracelet on his left wrist and goes by the nickname “Johnny.”
