Essen Healthcare Launches Virtual Care For Bronx Underserved, Aging

The Coronavirus crisis has imposed an addition burden on some of New York City’s most vulnerable and needy citizens: reduced access to routine care for patients of all ages with chronic illnesses, medical complexity, and health-related social needs.

In response to this crisis, Essen Healthcare, an integrated practice network with a 20+ year commitment to serving the healthcare needs of the underserved and elderly in the Bronx, is launching Essen VCare, a unique care platform that uses telephonic, remote and virtual care for all patients that need it, combined with select clinical offices and mobile health services open throughout the Bronx for those patients who need in-person evaluation and assessment.

“The Essen VCare Platform will provide access to healthcare professionals who can assess their broader healthcare needs, order medications and diagnostic tests, conduct video examinations, monitor their condition, triage for emergency services, consult with specialists, and provide care coordination,” noted Essen Healthcare Chief Medical Officer Sumir Sahgal, MD. “We will also support the Bronx community — patients, family members, healthcare and homecare staff — who need screening, testing and monitoring for coronavirus infection.”

Eligible patients on the Essen VCare Platform will also be able to enroll in the NYS Health Home program, which assigns them a dedicated Care Coordinator who will work with them to make sure they get the care and services they need to stay healthy.

Essen will be deploying physicians, nurse practitioners and physician assistants to work remotely to safeguard them from risk of CoViD-19 infection; additional professional staff have volunteered to continue to provide CoViD-19 screening services and in-person medical care when necessary.

The Essen VCare Platform is has already launched and tested with select Essen patients and will roll out more broadly to Essen patients and the general public over the next 10 days. VCare will accept patients covered by Medicaid, Medicare and most commercial insurances, and will have special discounted pricing for self-pay patients without coverage.
