This notification is sent pursuant to the Health Information Technology for Economic and Clinical Health Act.

On Labor day Sept 6th 2010 a robbery occurred at Counseling and Psychotherapy of Throggs Neck in Suite 220 of 3594 East Tremont Avenue, Bronx New York 10465 . A desktop computer, monitor, printer, keyboard and mouse were stolen, as well as, our office vandalized. On the computer was back up data of a patient database consisting of full names, social security numbers, date of birth, patient ID numbers, diagnosis and insurance information etc.

To protect yourself from the possibility of identity theft, we recommend that you immediately place a security alert on your credit files. A security alert means that before credit is issued to someone using your name, you will be personally called for your verification.. To place a security alert on your credit reports, contact any one of the three major credit bureaus. As soon as any credit bureau confirms your security alert, the other two credit bureaus will automatically be notified to place alerts on your credit report. This is done at no charge.

Using Experian as an example: call 888-397-3742. Follow the voice prompts: press #2, then #3, then #2 , then #1 and then #2.

By establishing your fraud alert, you will receive a follow-up letter that explains how to receive a free copy of your credit report. Examine your credit report closely for signs of fraud. Continue to monitor your credit reports, even though a fraud alert has been placed on your account. You may obtain a free copy of your credit report every 12 months by visiting, calling toll free 877-322-8228, or completing form here and mailing the form to Annual Credit Report Request Service, P.O. Box 1025281, Atlanta, GA 30348-5281.

Please know we take very seriously our role in protecting your personal health information, and are truly distressed by this breach. Please accept my sincerest apology for the stress and worry this incident has caused. We are doing everything possible to rectify the situation, including the following specific actions: The police were notified as well as state agencies. Locks were changed and employee conduct regarding security procedures were reviewed and modified. Letters will go out to all those affected. We will review computer security software to further ensure privacy.
